Maintaining any certification is harder than achieving it. One has to keep of all changes whether regulatory, external or internal. Once identified, processes and documentation must be revised and staff made aware and trained.
Periodic audits and random inspections are needed to identify problems and non conformances and rectify them.

It is either the documentation does not reflect the process or it is so detailed no one reads it. A problem with templated documentation is it requires customization, a task very few people relish doing so it falls by the side until the first external audit.

Sustained success requires a strong focused leadership that can unleash the potential of a motivated workforce that does the right things correctly the first time. Utopia, maybe but very much doable.

Documentation is always a problem, not so much in the control of revisions and content but in the depth and complexity of the content. In the 1980s the aviation industry worked on what was termed as Simple English. It was meant to provide a limited number of words that defined required actions without any confusion. Publications should be written in a clear and concise manner. Complex issues will require somewhat complex documentation but on the whole it should remain simple. A lot of people don't like to read, so it is better to offer them simple easy to read and com[prehend.

Objective setting is one of the hardest tasks that face management and Quality objectives are among the hardest.
Objectives in general should be realistic, achievable, measurable, in line with the organisations objectives and most importantly do not conflict with those of other departments. It is in the objective setting session that each manager has to outline the importance of each objective and how it is going to be achieved, validated and measured.
The ISO auditor or any QA auditor for that matter has only to ensure compliance.

Leadership provides the direction and the culture a company takes. An open and blame free management regime will do wonders in discovering and resolving issues in a timely manner. Communication within the organization and with clients is very important to provide understanding and buy in.

Senior management seems to be reluctant to organise during times of crisis. Something to do with comfort zones and an ever changing world and business environment.
Never waste a good crisis, it is the best time to reorganise, after all what do you have to lose, the business is already in trouble. This is the time to be creative and innovative to produce a new more efficient and productive structure

So what, not the end of the world, you look at the findings, analyse for root cause and put in a place a corrective action and implement it. Learn and move on to getting the right culture in the organisation.

Even medium sized companies might have to contract outside auditors to audit their Quality System, since technically their own QA can not audit themselves. However, companies who are audited by customers can take credit for these audits. Personally, as a minimum I will always audit the QMS whenever services are conducted along with other affected areas. If the QMS is robust and evidence of internal audits, NCRs closure and continuous improvement is shown and demonstrated then it is safe to accept that all services are up to the required standards.

Of the seven (7) steps the most important is the first one - Fully engage top management - without the buy in of top management and their championing the IS0 9001 effort, real progress and engagement of the staff will not occur.

ISO 9001 is like everything in life, is all about people, their actions and performance. We can map processes, write the best QMS manual and train as much as we want, nothing will happen until the staff are convinced. This conviction will only occur when top management is fully supportive of the effort.
